For your first appointment, please complete your intake paperwork before meeting with your therapist. If you prefer to complete the forms in our office, just let us know when scheduling your appointment.
When you arrive, make yourself comfortable in our waiting area. Your therapist will come greet you and bring you back for your session.
For Child Counseling Services
If you are seeking counseling for your child, the initial appointment is typically a parent or guardian session only. Unless you are told otherwise, your child will not attend the first session.
This first meeting allows us to:
Gather information about your child and concerns
Review intake paperwork and relevant history
Discuss goals for counseling
Develop an initial treatment plan
Answer any questions you may have about the counseling process
Our goal is to help you feel informed, supported, and comfortable as we begin working together.

Insurance & Payment Information
We accept several major insurance plans, including Blue Cross Blue Shield, Aetna, Cigna, United Behavioral Health (UBH), UMR, Optum, and Medicaid.
Please note that not all therapists are credentialed with every insurance company. We encourage you to review your therapist’s profile or contact our office to confirm coverage.
Out-of-Network Insurance
If we are not in-network with your insurance plan, we can provide a superbill for possible reimbursement. You may be responsible for the full cost of services until your benefits have been verified.
Before Your First Appointment
We recommend contacting your insurance company and asking:
What are my mental health benefits?
Do I have a deductible, and have I met any of it?
What will I owe for therapy sessions?
Do I have a copay or coinsurance?
While we file insurance claims as a courtesy, we cannot guarantee coverage or payment amounts. Your insurance company can provide the most accurate information about your benefits.
Self-Pay Options
If you choose not to use insurance, self-pay options are available. Rates vary by therapist, so please contact our office for current fees and payment information.
